Wedding Questionnaire & Add-On Form

Please let us know what your ceremony guest count is so that we can plan accordingly.
Please let us know where you are planning to stay on O'ahu so that we can assist accordingly. If you do not have a hotel booked yet you can also just let us know where you are planning on staying. ie. Waikiki, North Shore - Turtle Bay, etc.
Please say a few words about what your ideal beach or ceremony location would be like. You can say, bright turquoise waters, little crowds, close to our hotel, don't mind a drive, white sands and no rocks, grass near the ocean, short walk from parking area, or anything else that comes to mind.
Please list out everyone in your bridal party. Example: bride & groom, 1 maid of honor, 1 best man, 3 bridesmaids, 3 groomsmen, 1 ring bearer, 1 flower girl. If you don't have a bridal party please just say "bride and groom only".

Would you like a Live Musician? If you choose not to have a live musician we can play music of our speaker at no additional charge. *
Would you like to have a live musician at your Beach Ceremony? A live guitar player or violinist is a beautiful accent to your Hawaii Beach ceremony. We can also offer ukulele, but it does not sound as good or loud as acoustic guitar.
Please list which songs you'd like for your ceremony. You can choose up to 4 songs. 1. Processional (Bridal Party or groom) walk down the aisle 2. Brides Song (Bride walk down the aisle) 3. After you say "I do" (Recessional) 4. First Dance (if you want to do one on the beach). If you aren't sure you can email this list to your planner later.
Would you like Extra Time with your Photographer? (price includes editing) *
Included with your package is 1 hour of Professional Wedding Photography which will cover the ceremony plus about 30m. after the ceremony to do couple and family/group photos. Extra time with your photographer can be added on to cover getting ready at the hotel, more photos on the beach, more photos at a separate location, or photos at your dinner reception. Travel fees may apply. Prices will vary depending on the photographer. Your planner will get you a quote on extra time.
Wedding Videography
Would you like to add on a Professional Wedding Videographer or a Best Day Ever Highlight Video? For a Professional Videographer - Cost will vary depending on the company chosen and can range from $800-$3,000. This cost usually includes a professional video team, a highlight video, and a high-res DVD keepsake. /// As a less expensive option, we also do Best Day Ever Highlight Wedding videos for $250. Price includes one video about 2-4 minutes long, set to music. This does not include a DVD, just the web link to your video which will be hosted online for you to share with friends/family. We will send you the link of the video once it is completed. A video file may be emailed to you upon request only. If you'd like to add on a professional videographer please add in any notes here. You can say - ceremony coverage only, ceremony + dinner coverage needed, etc.
Ceremony Decor & Personalization
Included in your package is a natural vine aisle, hand-picked that morning. Would you like to add on flowers or any other additional decor? Please check all items that you'd like to include. You may also scroll down to the bottom of this page for sample photos. Please note, legally we are not allowed to bring more than 4 chairs on public beaches, generally these are used for older guests or parents, other guests stand behind or around the chairs. This is usually not a problem as the ceremony is not very long, about 20 minutes.
Would you like us to assist you with a few suggestions on where to take guests after your ceremony? We can assist with one of the following: lunch, dinner or luau. Please choose one, and list out any other specific details you have in mind. ie - I'd like to take guests to a luau that is near my hotel OR I'd like to take guests out to dinner after my ceremony, I'd prefer seafood and local inspired fresh foods, also I'd like a place that is kid-friendly OR I'd like a place for dinner that has great views and I'd like to spend about $100 per person (not including drinks).
